  • Under Section 2799B-6 of the Public Health Service Act (a.k.a. the No Surprises Act), health care providers and health care facilities are required to give patients who don’t have insurance or who are not using insurance an estimate of the bill for services, known as a "Good Faith Estimate." As a licensed medical professional I am required to provide you with a Good Faith Estimate (GFE) for therapy services.

✭ Under the law, you have the right to receive a GFE for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

✭ While it is not possible for me to know, in advance, how many therapy sessions may be necessary or appropriate for a given person, this form provides an estimate of the cost of services rendered that are provided per session.

✭ Your total cost of services will depend upon the number of sessions you attend, your individual circumstances, and the type and/or amount of services that are provided to you.

✭ If you receive a bill from any provider that is at least $400 more than your GFE, you can dispute the bill.

✭ You are not required to sign the GFE you but are encouraged to keep a copy for your records.
